Diablo 4 Season 12 Bloodied Items and Affixes Explained
 
Season 12 of Diablo IV introduces a new gear type called Bloodied items, and they completely change how aggressive gameplay feels. These items are tied to the Killstreak system, rewarding players who keep fighting without slowing down. Almost any drop in the game can appear as Bloodied, even if it's already Ancestral or Unique, so you can end up with gear that stacks multiple powerful bonuses as long as you maintain your momentum in combat Diablo 4 Items.

The whole system revolves around Killstreak tiers. When you kill enemies quickly enough, your streak climbs through several ranks: Killstreak, Carnage, Devastation, Bloodbath, and finally Massacre. The longer your streak continues, the stronger your character becomes, and the better the rewards tend to be.

For example, some armor rolls can increase movement speed for each Killstreak tier you reach, while others might reduce cooldowns or resource costs as your streak grows. If you stack several Rampage rolls across different armor slots, the effects add together, which can make your character dramatically faster and smoother during long combat chains.

Weapons can roll a different Bloodied affix called Feast. Instead of scaling with the tier of your Killstreak, Feast effects trigger based on how many enemies you've killed in the streak. One example shown for the season resets a random cooldown every certain number of kills. Other variations can grant buffs like temporary combat bonuses after reaching specific kill counts. Because these effects trigger at fixed numbers, they reward consistency: as long as you keep killing and don't let the streak drop, your weapon keeps activating powerful effects again and again.

Jewelry pieces—rings and amulets—can roll the Hunger affix. Hunger is less about combat power and more about improving rewards. These bonuses change what enemies drop during Killstreaks, such as increasing the chance for elites to drop certain materials or currency. When you stack several Hunger effects, long Killstreaks can become extremely profitable farming moments, especially when you're clearing dense areas full of monsters.

One of the biggest strengths of Bloodied items is how their effects combine. A character might use armor pieces stacked with Rampage for speed and resource efficiency, a weapon with a Feast effect that triggers buffs during long kill chains, and jewelry with Hunger bonuses that boost drops while the streak is active.
